Yeh document 28 October 2021 ki earlier SOP ke name-update process ki jagah leta hai aur requests ko alag-alag correction categories mein sort karta hai.
Har category ka apna paperwork hai. Agar kisi adult ka Aadhaar galat enter ho gaya hai, bhale hi unke paas original document sahi ho, toh unhe Proof of Identity ya Proof of Relationship ke saath Annexure B affidavit file karna hoga; minors applicable Annexure C use karenge. Shaadi ya talaq ke baad surname change karne ke liye Annexure D affidavit ke saath Proof of Identity chahiye hota hai.
Initials expand karna, naamon ka order badalna, middle ya last name add ya drop karna aur phonetically similar spellings ko correct karna minor changes maane jaate hain, identity change nahi. Poora naam replace karna ek major change hai: iske liye Gazette notification aur identity documents chahiye. Process karne se pehle, UIDAI har affidavit aur document ki authenticity check karta hai, aur applicants request Aadhaar Enrolment ya Update Centre par submit karte hain.
